Becker nevus

This is very common condition seen by Dermatologist in delhi in Skin clinics in Delhi.This condition was first reported by Becker in the late 1940s who described two young men developing localized hypermelanosis and hypertrichosis. It is not an uncommon condition, being reported in about 0.5% of young men. Lately, familial forms have also been reported.

Dermatologists see first appearance of nevus is in adolescence as an irregular hyperpigmented smooth macule on the shoulder, anterior chest or scapular region, although any part of the body may be involved. After a variable period, the macule grows in size to reach the size of a palm while acquiring thick, dark hairs, which may not precisely coincide with it. The surface becomes thickened and corrugated. Often lesions resembling acne vulgaris in different appear on the surface.

Usually single, Becker nevus may be multiple and bilateral. In females, abnormalities like polythelia, unilateral breast hypoplasia, spina bifida and limb asymmetry have been reported. Co-existence of underlying connective tissue nevi, smooth muscle hamartomas and structural abnormalities of bone and soft tissue is known. The male predominance, onset at or after puberty and the hypertricosis suggest that local androgen hypersensitivity may be an etiological factor.

Light microscopic examination in Skin n laser centre reveals a variable degree of epidermal hyperplasia and an increase in the number of basal melanocytes. Some authors have argued that the melanocytes are not disproportionately increased in number: Melanophages are found in the papillary dermis. Ultrastructural studies shown increased melanocytic activity and an increased number of melanosomes (sometimes giant melanosomes) in the melanocytes as well as keratinocytes. There are also an increased number of smooth muscles unrelated to blood vessels and hair follicles. The above findings suggest that Becker nevus is an epidermal nevus of the late onset “organoid type”.

Once present, these nevi stabilize after several months or years. Spontaneous regression is not known. Becker nevus is a benign condition and malignant transformation is extremely rare, although association with malignant melanoma has been reported. No treatment is required, although lasers have been used for cosmetic benefit.

The new term Becker nevus syndrome has been proposed for a phenotype characterized by the presence of a particular type of organoid epithlelial nevus showing hyperpigmentation, increased hairiness and hamartomatous augmentation of smooth muscles fibers, and other developmental defects such as ipsilateral hypoplasia of the breast and skeletal anomalies (including scoliosis, spina bifida occulta ipsilateral hypoplasia of a limb). The Becker nevus syndrome usually occurs sporadically. The associated anomalies tend to show a definite regional correspondence, suggesting a common origin from an early postzygotic mutation.
